Primary Purpose:

Under the direction of the Housing First Coordinator, provides individualized and flexible support to clients on the Housing First team by engaging them and helping them to reintegrate into the community. Assists clients to maximize their independence and potential.

 

 

Essential Duties:

                     Provides assertive outreach and engagement to clients as needed.

                     Assists clients with activities of daily living such as transportation, housekeeping, meal preparation, medication, shopping, laundry, or other hands-on assistance as needed.

                     Assists clients to access and navigate community resources.

                     Assists and supports clients in maintaining cooperative and effective relationships with their service providers and property management.

                     Assists clients in the development of life skills such as resolving interpersonal conflicts in their environment, maintaining housing, developing work skills, safety skills, social skills, and receiving and maintaining benefits.

                     Participates in planning and implementation of client assessments and action plans as needed.

                     Promotes recovery through role modeling and sharing experiences, and by providing hope, encouragement and connection to community.

                     Helps clients aspire to roles which emphasize their strengths by sharing personal experience.

                     Uses experience with personal recovery in assisting and supporting the team, and provides education and support regarding issues affecting the population.

                     Adheres to housing first and harm reduction model of peer advocacy.

                     Advocates on behalf of clients to ensure equity of treatment.

                     Keeps records of all client cases, and is responsible for ensuring that the records are entered into the clinic computer data-base, and for safeguarding clients' confidentiality and other rights.

                     Works with other Project staff in providing a coordinated system of care to the clients.

 

 

Other Duties:

Performs other related duties as assigned

Observes safety and security procedures; determines appropriate actions beyond guidelines; reports potentially unsafe conditions; uses equipment and materials properly.

Regular and reliable on-site attendance is required.

 

 

Minimum Qualification Requirements:

 

Skills/Knowledge:

         Good crisis intervention / de-escalation skills.

         Ability to relate effectively to difficult clients, including mentally ill, chemically dependent, and other homeless individuals.

         Ability to operate a vehicle with a valid  driver's license required.

         Must provide proof of automobile insurance and clean drivers abstract.

         Ability to work independently in an effective manner.

         Strong communication and interpersonal skills.

         Ability to work as a team player.

 

Education:

         High school diploma or GED equivalent required.

 

Experience:

One or more years of experience serving homeless persons with mental illness preferred.

Must have a previous history of homelessness, previous participation in mental health or substance abuse service systems as a recipient of services, and must also have a willingness to convey this experience.
